*Consulting on how to build toward and build-on the existing support team in place for you the participant in charge/transitioning in SDP: developing functional improvements in the management of one's multifaceted life, recruiting supports, training for direct professionals to support the person in the principles of the program - freedom, authority, support, responsibility and confirmation. Coordination of team members is rooted in the persons - person centered plan and how we as members can impact the outcomes as desired by the participant. Moral is critical and when real life happens to folks, people work best together when relationships are respectful and communication is reliable, a system is in place they can depend on to follow through.
Meaningful efforts, hard fought, hard knocks learning and key alliances have been made to bring about ground breaking change, and this has brought with it the necessary justice and attention to address the needs of folks and families thinking outside the box.

- The Program is offered by
- in the state of CA, the Department’s (DDS) Self-Determination Program (a government program) is one on offer, among the community based services, to eligible participants via contract through your Regional Center - that which provides advocacy oversight coordination planning, assessments and purchase of services; there are (21)twenty-one private, non-profit regional centers though out the state. In turn, they contract with local service providers, professionals, business owners, Facilitators, Vendors, advocates, direct supports professionals.

- As defined
- “Self-determination” means a voluntary delivery system consisting of a defined and comprehensive mix of services and supports, selected and directed by a participant through person-centered planning, a map to pursue one's objectives, a living document that informs the (IPP) Individuals Program Plan that which memorializes one's services/the contract with the state. Self-determination services and supports are designed to assist the participant to achieve personally defined outcomes in community settings that promote inclusion. The Self-Determination Program shall only fund services and supports provided pursuant to this division that the federal Centers for Medicare and Medicaid Services determines are eligible for federal financial participation.
- Participation in the Self-Determination Program is fully voluntary.
